Annual Appointment (Established Patients)

Annual Appointments: Our annual appointments work best when test results are already available, so you can review them with your provider. We are now scheduling annual appointments in two parts. You'll have an initial phone call to plan for lab work, and later, an in-person appointment to review the test results and make changes to your prescriptions and/or care plan.
